Control of proximal gastric bleeding through transgastric insertion of laparoscopic ports

Abstract

A 66-year-old man presented to a regional hospital with a 1-day history of haematemesis and malaena, associated with a haemoglobin drop from 138 g/L to 101 g/L. His past medical history included an open fundoplication performed 30 years prior, type 2 diabetes and chronic obstructive airways disease. On admission, he was commenced on an esomeprazole infusion and a gastroscopy was performed, which demonstrated Barrett's oesophagus and extensive blood clots in the antrum. However, the source was not identified as there was no active bleeding and not all the clot was cleared.
